Consultation & Evaluation
During your consultation, Jason will review your information and discuss if our weight loss program is right for you. After that, the next steps will include:
1. Get blood work drawn to complete the evaluation, which requires a one-time fee that includes labs and provider evaluation.
Recent labs may be used for a lesser fee.
2. Evaluation - Jason will help you map put your weight loss journey pending acceptable lab results.

Taking Medications
& Beyond
Semaglutide (Ozempic, Wegovy) & Tirzepatide (Mounjaro) for weight loss requires that individuals take once weekly subcutaneous injections.
Phentermine is a pill taken once daily in the morning.
These medications are the most popular but other options are also available. To be successful, all medications require you to making healthy changes to your diet, exercise, and lifestyle.

Having extensive personal experiences with obesity and weight loss, Jason will help guide you on your weight loss journey.

What's the Cost?
Initial Labs & Provider Evaluation - $129
recent labs may be used for eval fee of $79

Semaglutide (Ozempic, Wegovy)
Program Cost:
$70 per week
(if given weekly in office)
OR
$429 every 6 weeks
(4 weeks if at maximum dose of 2.4 mg)
Cost includes:

6 week supply of medication
(can be administered in office or supply provided for home administration)

Supplies (syringes, needles, alcohol wipes)

Titration of Medication
(cost will not increase as strength increases)

Monitoring & Coaching with Jason

Tirzepatide (Mounjaro, Zepbound)
Program Cost:
$549 - first 6 weeks
$649 - second 6 weeks
$749 - subsequent 6 weeks
(if dosage is increased)

Cost includes:

6 week supply of medication
(can be administered in office or supply provided for home administration)

Monitoring & Coaching with Jason

Phentermine (Adipex)
Monthly Visit fee $75
This includes the monthly visit as well as dispensing of
30 day supply of medication.
As phentermine is a controlled medication, patients are required to be
seen by Jason monthly. This will ensure any issues, obstacles, and plateaus
are discussed and also allows for dispensing of prescription.

Fees must be paid prior to ordering labs and receiving medications.
We do not submit claims to insurance. We are a cash practice,
and you must pay with cash, check, or card.
We also accept HSA and FSA payments.
